Intestinal obstruction during pregnancy carries significant fetal and maternal mortality. Cecal volvulus is an uncommon cause of bowel obstruction during pregnancy occurring when a poorly fixed cecum axially rotates or folds resulting in strangulation and ischemia. Without surgical intervention, cecal volvulus can lead to peritonitis, septic shock and ultimately death. Vague clinical symptoms, apprehension toward the use of radiographic studies in pregnancy and poor recognition of this condition often delay the diagnosis of cecal volvulus in this patient population. We present a case of acute cecal volvulus in a 19-year-old, pregnant female with right upper quadrant pain successfully managed with caesarian section delivery and right hemicolectomy.
